Please read this document carefully before participating in any Easy Day Boxing Gym program or event.

Acknowledgement of Risk

I understand that participating in activities hosted, organized, sponsored, instructed, or facilitated by Easy Day Boxing Gym (“Easy Day”) involves inherent risks, including but not limited to:

● Boxing and kickboxing

● Strength and conditioning

● Personal training

● Group fitness classes

● Pilates, yoga, stretching, mobility, dance, cycling/spin, and boot camps

● Outdoor workouts

● Community fitness events

● Charity events

● Wellness workshops

● Walking, hiking, and running events

● Sporting competitions and challenges

● Special events and pop-up experiences

● Any other activity hosted or promoted by Easy Day Boxing Gym

I understand these activities may involve physical exertion that could result in muscle soreness, strains, sprains, broken bones, concussion, illness, permanent disability, property damage, or death.

I voluntarily choose to participate despite these known and unknown risks.

Health Statement

I certify that I am physically able to participate.

I understand it is my responsibility to consult my physician before beginning any exercise program if I have concerns regarding my health.

I agree to immediately stop participating and notify an instructor if I experience dizziness, chest pain, shortness of breath, fainting, or any unusual symptoms.

Assumption of Responsibility

I agree to:

● Follow all instructor directions.

● Use equipment responsibly.

● Wear appropriate athletic clothing and footwear when applicable.

● Inform staff of any medical limitations that may affect my participation.

● Conduct myself respectfully toward instructors, staff, volunteers, participants, and property.

Easy Day reserves the right to remove any participant whose behavior is unsafe or disruptive.

Release of Liability

To the fullest extent permitted by law, I voluntarily release and forever discharge Easy Day Boxing Gym, its owners, employees, contractors, instructors, volunteers, partners, sponsors, landlords, event hosts, and affiliated organizations from any and all liability arising from my participation.

This includes claims for:

● Personal injury

● Illness

● Property damage

● Economic loss

● Wrongful death

whether arising from ordinary negligence or otherwise, except where prohibited by applicable law.

Outdoor Events

I understand that outdoor activities may involve:

● Uneven terrain

● Traffic

● Weather conditions

● Wildlife

● Other participants

● Public spaces

I voluntarily assume these additional risks.

Equipment

I understand that I may use boxing equipment, resistance equipment, weights, bicycles, mats, steps, bands, and other fitness equipment.

I agree to inspect any equipment before use and notify staff immediately if I believe equipment is unsafe.

Photography & Video Release

I understand that photographs and video may be taken during Easy Day activities.

Unless I notify Easy Day in writing before participating, I grant permission for my image, likeness, voice, and name to be used for promotional, educational, social media, website, advertising, and marketing purposes without compensation.

Medical Emergency Authorization

In the event of a medical emergency, I authorize Easy Day to obtain emergency medical treatment on my behalf if I am unable to do so.

I understand that I am responsible for any medical expenses incurred.

Minors

Participants under eighteen (18) years of age must have this waiver signed by a parent or legal guardian.

Governing Law

This Agreement shall be governed by the laws of the State of Michigan.

If any portion of this Agreement is determined to be unenforceable, the remaining provisions shall remain in full force and effect.

Electronic Signature Agreement

By checking the box below and signing electronically or manually, I acknowledge that:

● I have carefully read this entire document.

● I understand its contents.

● I understand that I am giving up certain legal rights, including the right to sue.

● I voluntarily agree to these terms.
